Schizophrenia, the archetypal psychotic disorder, is a chronic, currently incurable, prevalent, and devastating syndrome. Clinical lore and the author's experience suggest that sleep complaints frequently accompany schizophrenia and other psychotic disorders, just as they do many other psychiatric disorders. Polysomnographic studies of sleep in psychotic disorders have examined schizophrenia and schizoaffective disorder exclusively. The only established risk factors for the development of schizophrenia are: having first-degree relatives with schizophrenia, being raised in an urban environment, being an immigrant, being exposed to a range of obstetric complications including infections, and being born in late winter or early spring. The available literature on the treatment of obstructive sleep apnea (OSA) in patients with psychotic disorders consists of four case reports of patients with either schizophrenia or schizoaffective disorder and sleep-related breathing disorders treated with continuous positive airway pressure (CPAP) therapy.
