Conocimiento de los problemas metabólicos de los pacientes con trastorno bipolar: encuesta a psiquiatras europeos

Resumen

Objetivo

Evaluación mediante encuesta en internet del conocimiento del síndrome metabólico de los psiquiatras europeos y su influencia en el tratamiento del trastorno bipolar.

Materiales y métodos

Psiquiatras del Reino Unido, Francia, Alemania, España, y Italia fueron encuestados desde abril a junio de 2006. Los criterios de elegibilidad fueron 4-30 años de ejercicio de la profesión, ≥50% del tiempo dedicado a la atención directa del paciente y haber visto ≥10 pacientes con trastorno bipolar durante el mes anterior. Los datos agregados se ponderaron para representar a la población de médicos en ejercicio de cada país.

Resultados

El 56% de los 718 que respondieron había diagnosticado el síndrome metabólico. Los que respondieron afirmaron que la prevalencia del síndrome metabólico era mayor en los pacientes con trastromo bipolar (25,1%) que en la población general (20%). El 72% consideraba que el síndrome metabólico suponía un riesgo significativo para la salud, que exigía control/tratamiento, y estaban muy preocupados con los efectos adversos de la medicación bipolar, como aumento de peso, deterioro cognitivo e intolerancia a la glucosa. Los que respondieron a la encuesta reconocieron diferencias claras entre los distintos psicotrópicos en la propensión a inducir efectos metabólicos adversos. El 65% de los que respondieron indicó que ellos habían hecho cambios en la entrevista y en el control en los tres últimos años como consecuencia de los problemas metabólicos.

Conclusiones

Los psiquiatras europeos consideran el síndrome metabólico sumamente frecuente en la población general y en pacientes con trastromo bipolar; dos terceras partes de ellos cambiaron su tratamiento de los pacientes con trastromo bipolar por problemas metabólicos.
