Case 37 - Oropharyngeal Synovial Sarcoma

Summary

A 17-year-old male presented with a history of sore throat, accompanied by increasing dysphagia. A large nonulcerated mass of the right tonsil was evident at physical examination. CT scans revealed a tumor of the right palatine tonsil, almost obliterating the oropharyngeal space, and a neck mass, which was interpreted as lymph node involvement (Figure 1). After an incisional biopsy of the tonsil, which showed only hyperplastic lymphoid tissue, an incisional biopsy of the neck mass was performed. Treatment included tracheotomy, followed by cycles of chemotherapy and radiotherapy, which resulted in a marked decrease of the tonsillar and neck masses. A relapse of the tonsillar lesion occurred 24 months after diagnosis.
